Unfulfilled pregnancy wish – Initial questions and answers

At what point does one speak of involuntary childlessness?
We speak of involuntary childlessness, when after 1 – 2 years of unprotected intercourse pregnancy has not been achieved.

Who does this happen to?
Every sixth marriage remains childless although the couple wants children; that means, that in Germany about 2 million couples live with unfulfilled parenthood dreams.

What possible causes are there, when pregnancy does not occur?
The reasons are varied. Unrealized pregnancy is a problem that equally affects both partners: Statistically the reasons are divided 30 – 40 % each for husband and wife. For 15 – 30 % of the affected couples both partners are the cause and in 5 – 10 % of the cases no cause can be identified.

What can be done about it?
The treatment targets the causes childlessness. The more exactly they can be diagnosed, the better the chances that it can be overcome with medical treatment. Sometimes only the correction of excessive hormones is necessary, e.g. thyroid function or high production of the male hormone (Androgen).

If the maturation of the female egg cell is disrupted, a relatively simple hormone stimulation will be initiated. Conception is then achieved in the normal way. If number and movement of the man's sperm are also limited, the hormone therapy will be combined with artificial insemination (IUI): In this case the sperm will be injected directly into the uterus through a thin tube at ovulation.
If the fallopian tubes are closed and can't be opened surgically, immobile or completely missing, conception outside the body, that means an In-vitro-Fertilization (IVF), is necessary. Along with hormone stimulation, a minor outpatient surgery is necessary to extract mature egg cells.
